It seems like a freak occurrence that would happen once in a very rare while. I understand half-orcs as they would likely take female prisoners but OGRES? They seem too stupid to conduct raids & take women. Aside from that, the woman giving birth would likely need a massive c-section & not even survive. What gives? Why is this so common?

In Arcanum, you can find out that Gnomes are actually breeding half ogres as part of a conspiracy, which involves some pretty heinous things like kidnapping and rape to get the science project going. They document that most human women do not survive the birth, and they also do research about human male/ogre female pairings. I don't remember all the details, though. They are so prominent in the Arcanum setting because Gnomes have been doing this for quite a long time, mostly to combat their disadvantage of being small and easily pushed around, half ogre bodyguards are basically a Gnome creation so that it doesn't happen.

They were an early option in D&D from Dragon Magazine, written by Gygax himself, but even with Metzer's revision. Still pretty unplayable.Turned up in 2E as a option in the monster looseleaf, Book of Humanoids and even in the 1995 Skills & Powers 2.5E books.They were overpowered for combat monsters and sucked at everything else.

presumably there are a few reasons
firstly, most half-ogres you see in settings where they are common are likely are the result of two half ogres reproducing, with no actual ogres involved. secondly, it would likely be that either human women can naturally birth half-ogres easily, or it is female ogres forcing men. thirdly, half-ogres with their immense strength and passable intelligence likely have few problems surviving in any society that tolerates them.
