As of January 2024, Quartz began covering FFS and still does. I'm not sure if they still will in 2026, but I have mine scheduled for the end of the year. You can obtain insurance through Quartz by working a job that offers it to employees (working for the state of Wisconsin, city of Madison, other municipal governments, Dave Jones, UW Health, SSM Health, UW Madison, MATC, and more) marrying someone who has it, being under 26 and having parents who have it, or doing what I did to obtain it through the marketplace with a massive Obamacare subsidy. If you go to an in-network surgeon like Dr. Garland, they will cover it all and not even ask for a therapist letter. Here's how much mine is gonna cost vs how much I actually have to pay for it.

hopefully this gets changed, a bit silly that something like THIS is covered at all.
