My index finger hurts like a motherfucker and so does the palm of my thumb but not as much. I hold both a mechanical pencil and digital stylus like this by the way. I had a problem before with the grip but I cant think of any reason why my index would hurt. Any ideas?

dynamically switch between different grips to make different kind of strokes and reduce strain. using the same grip all the time will cause overuse injuries.

Get a tablet stand and draw with your elbows and shoulders more often. Keep a loose grip on your pen. I use a writing grip which is the same as my drawing grip. If you're still having pain or inflammation it's probably a good idea to go see a physical therapist.

You are pushing too hard. Likely due to the choice of lead being too light. I just saw someones art in another thread. You have to stop being afraid to make a mess. Use a darker lead, don't press as hard, your finger will get stronger.
